What is Family Medicine?

Family medicine is a medical specialty and a division of primary care that provides continuing and comprehensive healthcare for the individual and family across all ages, genders, diseases, and body organs. This medical specialty is based on knowledge of the patient in the context of the family and the community, and it emphasizes disease prevention and health promotion. 

Your Family doctor will:

Guidance of Family Physician

With a family physician on your side, you have a partner to guide you and help you manage your health. Family medicine practitioners develop personal relationships with their patients and build a history with them. 

Not only are they a great resource for preventive care, but your relationship with them and their insight into your health history are crucial when irregular symptoms - physical or emotional - point to potential diseases and conditions. And when a specialist's care is needed, your family medicine practitioner will serve as an advocate for you and coordinate your care with the needed specialist, helping you navigate new information and recommendations.

Complex Case Management

The Case Management service is a collaborative process in which patients and clinicians identify and discuss ways to improve the patient's health, wellbeing, and quality of life through the development and implementation of a holistic care plan.

This proactive approach enables the multi-disciplinary team, including a care coordinator, to identify and understand patients’ health needs through better understanding of their health and self-management goals, helping to reduce unnecessary hospital admissions, and make better use of resources.

  • Support from a multi-disciplinary team to develop a personalized and holistic care plan to improve your health and management of any health conditions.
  • Help you set goals that are important to you and your health with support to achieve them.
  • Work with your care coordinator, assigned family medicine physician and specialty doctors to coordinate your care.

Typically, case management is for people with two or more chronic health conditions. The service is for registered PHCC patients with multiple chronic conditions that require additional support. Eligible patients will be directly contacted by their HC to enroll in the service. The service will initially be available at Al Wajba, Umm Slal, West Bay and Al Wakra Health Centers with plans to roll out the service at additional health centers soon.
