PHCC’s Corporate Strategic Plan 2019-2023 was launched in April 2019. 

The development of the Corporate Strategy was a collaborative effort of many people across the organization and our health partners. It builds on key platforms such as the Qatar National Vision 2030, and the new National Health Strategy (NHS) 2018-2022: Our Health, Our Future. It aims to place primary health care services as the foundation of the health system in Qatar.

Our Strategy is evidence that we are committed to putting our patients at the center of health care delivery, enabling them to live healthy, productive lives, and to control the risk factors that affect their health, particularly in the national context of rising challenges of chronic disease, road accidents and injuries. We understand the importance of a strong primary care infrastructure and a family medicine model of care in delivering better value and affordable health care services. We know how important it is to employ and develop excellent and dedicated staff who are focused on patient safety, patient experience and high-quality patient services. Therefore, our Corporate Strategic Plan aims to set goals in these areas over the next five years, and why it is important to the overall health and wellbeing of our society.

A key starting point in the development of the Corporate Strategic Plan was to refresh our Vision, Mission and Values.

Why We Need a Strategy?

Since the publication of the first NHS 2011-2016, there has been recognition of the importance of primary care in achieving the human health and development goals of the National Vision 2030, and the United Nations Sustainable Development Goals. The delivery of the National Primary Health Care Strategy 2013-2018 established primary care as the “foundation of the health care system” and facilitated many advancements to the services we provide to our patients and families.

Healthy children and adolescents.

Healthy women leading to healthy pregnancies.

Healthy and safe employees.

Mental health and wellbeing.

Improved health for people with multiple chronic conditions.

Health and wellbeing for people with special needs.

Healthy ageing.

The PHCC Corporate Strategic Plan is well aligned with NHS 2018-2022 with both strategies envisioning a future model of care that focuses on health promotion, prevention and wellbeing in the hopes of tackling Qatar’s growing health challenges.

The latest National Health Strategy highlighted the challenges still facing the health of Qatar’s population:

of Qatari adults have low levels of physical activity

Our Strategic Framework

To address these health inequalities and improve services for our patients most in need, PHCC’s first corporate strategic plan sets out six strategic priority areas and 20 strategic goals which we will focus on delivering over the next five years to achieve our vision, mission and national direction.

Our strategic framework is founded on the health improvement principles of IHI’s Better Health, Better Care and Better Value model.

The two major service programs aligned to the seven priority population groups set out in NHS 2018-2022 are:

Implementing high-quality integrated Family Medicine Model of care.

A focus on Preventive Health.

The four system enablers are the broader national or corporate level interventions that support effective implementation of the two patient-centered programs. The Better Health, Better Care and Better Value propositions underpin the six strategic priority areas.

Our Service Vision and Integrated Care

Our service vision is to bring more health care services delivered in community locations, closer to the homes of individuals and families. Through the Family Medicine Model, we will develop the desired components over the next five years.

PHCC’s Corporate Strategic Plan represents the required shift in balance to enhance preventative care, with a strong approach in the overall health of the population. We intend to realize our vision by engaging in more integrated care opportunities and increased access to comprehensive services in community settings, and through collaborating with our health colleagues and key stakeholders to collectively impact on improved health outcomes for the people of Qatar. As the first step to health care, PHCC has a central role to play in realizing this vision.

Integrated care must improve timely access across the healthcare continuum in multiple settings and levels of care – from prevention right through to follow-up and surveillance. We will offer care across the primary and acute sectors to best support the patient’s health needs, through use of multi-disciplinary clinical teams, and in health locations where patients can be cared for in the community, closer to their homes.

Timely Unplanned Care

Engage and inform people with the right information, so they can take greater ownership of their health and wellness needs.

Proactive Care

When they need care, ensure all patients have named physician and health team to support them.

Routine Planned Care

Provide patient centered, comprehensive, and coordinated primary care services in our expanding health centers.

Integrated Across the Health System

Ensure more health care services are delivered in community location, closer to homes and workplaces of our patients.

Health Needs Assessment

Goal

The population health profiling aims at providing a better understanding of PHCC’s targeted population health needs, risk factors, and prevalence of diseases. The data will support better service design, implementation, and resource allocation to respond to the population health needs.

Objectives of the Health Needs Assessment

To assess the prevalence of chronic NCDs among the population registered at PHCC facilities.

To assess the prevalence of depression and anxiety among the registered population at PHCC health facilities.

To provide up-to-date information for assessing the modifiable behavioral risk factors and the metabolic risk factors for developing chronic NCDs among the population registered at PHCC health facilities in Qatar.

To provide up-to-date information for assessing the situation of new-borns, children and women registered at primary health care facilities in Qatar.

To understand the incidence of the communicable diseases reported at the PHCC health centers.
